Application will be ready after JULY 1 - Complete the Meal Application - one per family

- Proof of Residency (Energy, rent, water bill w/legal guardian name & service location)
- Shot Records
- Physical Examination
- Current Dietary/Nutrition Needs (must have doctor's documentation)
- Current Allergies (must have doctor's documentation)
- Papers concerning legal guardianship, custody papers (if applicable)
