Re:  Kindergarten Registration

Kindergarten registration has begun for the 2006-2007 school  year at Margaret Daniel Primary School.  If you have a child that will be 5 years old on or before September 15, 2006, you may come by the school office and fill out registration papers.  We encourage all prospective kindergarten students to be registered as soon as possible so that we may know the number of kindergarten teachers that are needed for next year.  Parents will need to bring the following to register your chid: 

·        Current shot/Immunization record

·        Copy of birth certificate or other document indicating child’s age

·        Social Security card

·        Documentation of a physical exam performed by health care professional

Your child should have these shots:

·        4 DTaP/DTP(diphtheria, tetanus, accellular Pertussis) shots

·        3 IPV/OPV (polio) shots

·        2 MMR (measles, mumps, rubella) shots

·        1 Varicella (chicken pox) shot

The last vaccine should be taken after the 4^th birthday.

Hours for registering your child at Margaret Daniel Primary School are 9 – 2 each day and from 5 -7 p.m. on Thursday, March 23^rd.  You may call the school office at 870-898-3711 for more information.
